📢 Adolescent Immunisation Program Information
Protect your child from serious diseases with free vaccinations for Year 7 and Year 10 students!
✅ Why Vaccinate?
Vaccination is the most effective way to protect your child from diseases that can cause serious harm.
✅ Important Dates:
Year 10’s: Tuesday, 11 March
Year 7’s: Tuesday, 12 August
✅ How It Works:
Fully trained immunisation teams will visit schools to provide these free vaccines.
✅ Consent is Essential!
Your child will only be vaccinated if you provide consent.
Complete the online consent form here to ensure your child receives their immunisations this school year.
📄 Prefer paper?
Hard copy consent forms are available from your child’s school office. Complete and return them ASAP!
💡 Additional Information:
Missed a vaccine at school? Visit your GP, community immunisation clinic, Aboriginal medical service, or participating pharmacy for a catch-up vaccine.
For children under the care of the Department of Communities, contact your dependent’s case worker to complete the form.
